ONLY THREE THINGS ARE NEEDED TO DO YOUR EVALUATION…
1. IF YOU TOOK THE BREATH OR BLOOD TEST I MUST HAVE PROOF OF THE RESULTS.
The NOTICE OF SUMMARY SUSPENSION is best. It’s on the other side of your receipt to drive. If you refused the breathalyzer bring your tickets. The easiest way to make sure that I have the papers I need is to bring ALL the papers they gave you when you got arrested.
2. DRIVING RECORD FOR COURT PURPOSES.
Go to a driver’s license office and ask for your “Driving Record for Court Purposes”. The driving record thy give you has to say “Court Purposes” in the upper left hand corner. They will charge you $12 and you will have to provide identification or they won’t give it to you.
3. EVALUATION FEE.
The normal fee is $100 but if your income is low enough, or if you are out of work, or on food stamps or on public aid, etc. you may qualify for a free evaluation. If you think you might qualify for the free evaluation then proof of income will be necessary. Ask me about it...and there’s no need to be shy. I don’t care if you pay me or the state pays me. |
